New Client Steps

Welcome! Let's get you onboarded. 

There are 3 steps to get things rolling with your engagement for services:

  1. Pay your Good Faith Deposit

  2. Fill out the New Client Intake & Authorization Form

  3. Upload your documentation to the secure Client Portal so staff can assemble your file

What documents are needed?

As a new client there are a number of documents we need to collect from you - not only to build out your internal file to make the process as efficient as possible but also as required by the due diligence regulations as set forth by the IRS, state departments, and as required for the type of work we do. 

Thank you for your diligence in gathering the items we need in order to best serve you as a client!

Always required from every client:

  • Government-issued photo ID (driver's license or passport)

  • Social Security card or document showing SSN

  • Last 3 years of federal and state tax returns

  • Current year tax documents (W-2s, 1099s, K-1s, etc.)

  • Any IRS or state notices received in the past 12 months

 

If applicable — personal:

  • Divorce decree, custody agreement, or parenting plan (if applicable to dependent claims)

  • Prior year extension confirmation (if an extension was filed)

 

If applicable — business:

  • EIN Letter from the IRS (CP 575)

  • Articles of Incorporation or Organization

  • Business ownership/operating agreement

  • QBO access (accountant invite to jacki@ascendance-consulting.com)

  • Payroll software access or payroll ledger

  • Minnesota State Tax ID

  • Minnesota Unemployment Insurance ID

  • Minnesota Paid Leave ID

  • Sales tax account information (if applicable)

  • Prior year business tax return if filed elsewhere

 

If applicable — bookkeeping cleanup:

  • Bank statements for all business accounts (for the period being cleaned up)

  • Credit card statements for all business accounts

  • Any existing chart of accounts or bookkeeping records
